She is obviously extremely emotional and not level-headed. People like this do CRAZY things, so just stop all contact with her, especially since she knows where you live.
Now I am worried for you. My DH would have had my head if this was me...he is a worrier too.
Ugh, she sounds crazy, I'm sorry you have to deal with her. I would stop responding to her no matter what she writes. Don't say anything to her about the negative feed back that she left for you, just completely ignore her. Eventually she should get tired of sending messages.

Not sure if you knew this, OP, but the seller has another shop as well, and it looks like this isn't the first time she's gone off her rocker over what she considers "unsatisfactory" feedback.
From her other shop's feedback page:
there is only one problem - I didnt receive an item after 30 days of waiting and when I open a dispute in paypal a seller begins to insult me ​​and accuse of cheating. The seller didnt provide me any confrmation of delivery and have no documnts from the post office. The the seller is not interested in resolving the dispute and immediately began to talk aggressively

She shouldn't have her credit card info. Most transactions are done through paypal or through Etsy's CC processing. I've never had access to any of the credit card info from any of my buyers.

Can you call Etsy and explain the seriousness of the issue to them on the phone? I feel like they don't get it and need more info. Agree with The Foozzler that this was just a form letter.

I don't think they have a phone number! I emailed that guy back and said I'm concerned about her having my address etc and I copy/pasted her most recent insane message so hopefully he will get back to me.
Trudy, I think it's time to report this woman to her local police. Maybe contact your PD and see about how it works across state lines, but both states have cyberharassment laws. She is harassing you and now she's making veiled threats.

DH works in the building with Etsy. He is going to check the building's directory and see if the office has a number. I am sure there is more than one, but they are in Brooklyn and might be able to help push the matter along to the right phone number?
I mean, this woman is crazy. Clearly. But I'm not sure i would call the police. Seems a bit much. But, if it makes you more comfortable, you should!
Filing a police report or complaint isn't really about getting them to do anything at this point. It's more about making sure that in the unlikely event that this esclates and the woman does something to harm trudge it is documented that there is already a history of harrassment. If she does not stop, a police report now will make it easier to file a restraining order or even press charges later.
