Well, sort of...not that it was my fault, per se. I was just trying to do a good job. Long story short, for one of the projects I've been working on, I noticed a lot of discrepancies between what was in the clinical study report and what was in the corresponding data tables. When I originally brought this up, I was told by the client not to worry about it at the moment because everything was going to be checked over by a statistician. After the statistician supposedly checked things, there were still all sorts of discrepancies and some new questions, so we sent everything back to the client with queries. That was over a month ago. We were supposed to get responses back late last week, the project manager asked the client for an update. The response was, "We hired a new statistician...so maybe next week..."
The kind of sad thing here is that I am by no means any kind of statistical whiz.

Well, people are expected to do their jobs, and it's not your fault if they weren't doing it properly. I can see why you would feel bad but it truly wasn't any fault of yours. Hopefully the new person does a much better job!

Oh, I don't really feel bad exactly, just a bit surprised. In fact, I'm not even sure he was fired. The response was a bit vague, he could have quit, but considering everything, I'm guessing he was let go.
Don't feel bad. You are good at your job and that person obviously wasn't good at their own job. I hate to be blunt but good for you catching the mistakes!

Good for you! It sounds like it was the person's actions, not yours, that got them fired. From a business integrity standpoint, you absolutely did the right thing.
