My girlfriends were spread out throughout the Great Lakes region. We met up for a weekend at Put-in-Bay, rented a home, and did some major day drinking (which turned into night drinking and dancing) each day. It was fab.
This was back in '07, and everytime I hear Rhianna's "Umbrella" I think about that awesome weekend!
We went to Oktoberfest in Big Bear, Ca. Rented a cabin for the weekend so we could bring in our own food and booze, and got one that had a hot tub and fireplace.

It was the Thursday before our wedding.
During the day, my bridesmaids surprised me and took me lingerie shopping, which was actually fun. Then a slightly larger group of only women met up for pre-dinner drinks and small gifts a my friend's apartment. That was followed by dinner at a restaurant in downtown Milwaukee, and a couple of my guy friends joined us for that.
H and his dudes had gone out to dinner, and afterward we all met up at a swanky bar.
It was very low key. All my bridesmaids and most of our guests were already traveling far. Nobody could have afforded a destination party. Also, I despise destination bachelor/ette parties/weekends.
Drinks at my sister's apartment in SF followed by dinner out and more drinks at a couple bars, then we stayed at a hotel in the city. It was just my bridesmaids and I, who all lived within a couple hours of SF (one bridesmaid lived cross-country and couldn't attend).
It was in the town where I went to undergrad (where my sister and bridesmaids all still lived). We went to a pole dancing class (lol), then had dinner at my sister's apartment and then barhopping downtown.

Here in NY to make it easy for people flying in from both the west coast and London.
Friday night - dinner and drinks with my MOH Saturday - morning: brunch; afternoon: Vosges chocolate set up a chocolate tasting with Champagne at my friend's art gallery; evening: cooking class/dinner Sunday - my shower at a tea salon
Girl's weekend in southwest Michigan. Nothing crazy. We did this for all the girls in my circle of friends. We all got married in our late 30's/early 40's, so we were over the crazy night out.
DC, I lived in Arlington. Only my FSIL and her friend came into town. We went out to eat at Cafe Asia and then went to a few bars, Cafe Citron, Mr. Smith's, some random hookah bar. Spent the night at a hotel in Arlington and then went to brunch at Jaleo.
It was fairly low key and not too pricey which was fine by me, I didn't want anyone going broke to try to pay for it.
Epic girls night Wednesday before the wedding. 4 ppl total. Went on a 2 hr sailboat trip w/ bubbly wine, dinner on a rooftop, different bars. There were no ridic outfits other than matching heart shaped sunglasses and there was some celeb staying at our hotel so we asked the paparazzi to take our picture with my camera. So fun.
Side note, my maid of honor fell asleep in my bed. She was awoken by room service delivering a club sandwich to my now husband who had gotten back from his bachelor party, but fallen asleep on the couch since we were occupying the bed. She ate it and went back to hers

We did a cupcake tour in Boston (self-led - my sister found a bunch of cupcake shops and we took a limo between them) and then went to dinner and drinks closer to home. We live about an hour outside the city.
I got blitzed out of my mind. All I remember was as soon as I got to the next bar I went to the bathroom to vomit so I could "keep going." I also remember seeing a male Go Go dancer and saying in a cartoon voice "He has a mother!" over and over again.

Me, my best friend, my sister and my mom at mom and dad's house...we ordered take out Japanese, drank wine, and watched movies. Later on I played "dress up" with my gown to make sure it was ready for the morning and do a quick make-up trial, and we took pictures with my 5 month old niece (since my sister wasn't bringing her to the wedding).
