We got a text today saying 90% of our data had been used. I immediately turned off the 4g on my phone. Dh and I checked our account online, and my data usage was really high.
3 hours later, I get a message that we are over our data. Check the online account, and it's clearly data attributed to my phone, which has been sitting on the counter charging for 3 hours, untouched and 4g off.
Personal hotspot, push notifications, auto updates are all turned off (I usually keep all these off) .
Anyone have any ideas what could be sucking up the data?

Check the Facebook app if you have it, one of their latest updates has videos to auto play and will eat up data (turn it to play on wifi only.) We have AT&T and have my step daughter's phone set to just below her data plan so it shuts off. She was constantly going over as previous person said, there is a delay in the data reporting.

I don't know if iPhone has this, but on my android phone in the settings I can view data usage broken down by app/service. Maybe iOS has something similar.

You can. You have to manually reset it each month though. It's buried under setting.

This. Reset it and then in a few days you can take a look at what's eating up your data. Facebook and background app refresh tend to be big ones. And there's usually a 12-24 hr delay on the data usage alerts, so unfortunately you had already gone over by the time you turned it off.

How do you adjust the auto play on the app? It's probably right in my face, but I don't see it.
