We had to cancel our honeymoon due to a medical emergency. We received a full refund from both the cruise line and airline. I seem to remember the airline wanted to give us a voucher for future travel, but we pressed for a full refund. The $11,400 trip we never took ended up costing about $140.

I would call the insurance company, tell them the situation, ask what documentation they need from you and whether they take care of cancellations or whether you need to. We had to cancel a trip, literally as the flight was boarding, a few years ago bc my FIL passed away. I called the insurance company, told them what happened and the entire cost of the trip was refunded to me within 2-3 days. I didn't have to deal with anyone other than them.
We had to cancel an international trip in 2011 after my dad passed away. I think we used Travel Insured as well. Unfortunately I don't remember the details, but I do know that the process went really smoothly. I just faxed them some paperwork, and we got a check in the mail. They had excellent customer service. I know I was too out of it that week to have completed any task that was actually challenging.

Call insurance or go online to start the claim. They usually have their own medical statement form. If you insured the air tickets the insurance normally gives you the cost back if they were non refundable. When I've cancelled I've never known if o was going to travel again within the time frame the voucher is valid for from the ticket purchase date. If you get the voucher insurance may cover the change or penalty. Would have to check your policy.
If you didn't insure them, at least with United in the fine print they have a medical refund minus a $50 processing fee.
