I'm pretty sure Rx's that I have that were written by a PA do say "Dr." on the label, but I don't think I've ever thought about what the appropriate salutation is. A PA degree is a masters, not a doctorate. The PAs at my PCP's office just go by/introduce themselves by first name, so that is what I'd call them. Can you just start it, "Hi John..."?
I don't think most have a doctorate degree, so calling them Dr. is inappropriate.
The only PAs I deal with are at my orthopod (he has 3, all are awesome), and they all get called by their first name. That is how they were introduced to me, and what I've always called them that when dealing with them.

This has been my experience as well. Also my sister is a PA and everyone just calls her by her first name.
