I'd be freaking but with additional info, I think you're good. You were on your way back from the doctor's appointment and soup or something sounded good. Or a milk shake for your throat. Or soda to calm your stomach. Whatever. There is no way he is going to be all "Well, her shoes seemed AWFULLY FANCY for someone who is 'sick.'"

If you need to cover this, you could say that it was an appointment for a well women exam. You took the full day as sick day b/c you see a doctor that is not near and often don't run on time/on schedule. Easier to take the whole day then to battle/rush in traffic.
The phrase "well women" will put most people in an uncomfortable state and will not continue the conversation. Rather this moment of awkwardness than lingering questions of you lying/covering up something.
