My four old has been fully potty trained for about a year. In the past two weeks she has had multiple poop accidents.
We have started asking her if she needs to go and she says no. Then has an accident a few minutes later.
She has also been holding it as long as she can so that if she tries to go on the potty it hurts and can take multiple tries in the day to actually go.
Today I was feeding her sister, she came over and we were talking. Then I noticed a smell and asked her if she was stinky. She said no and tan off. I had to stop feeding baby to caych her and clean her up for the second accident of the day.

We had this happen in the fall. DD checked out at the Dr, we chalked it up to a lot of change going on in her life. The Dr recommended Culturelle just in case there were physical issues, and we went back to a sticker chart and tons of encouragement. She got over it after a couple of months and hasn't had an accident since November
Ditto the above. Constipation can basically undo potty training so you gotta treat it and prevent it. Ease up on the milk and cheese and feed more oatmeal, blueberries, apple juice, etc.
I'd also go back to sitting her on the potty when you think it's time or telling her to "go try" - not just ask and take her at her word. Also make sure she has real privacy in the bathroom and can be in there as long as needed.
