It's pretty clear. They asked for you to provide a recipe. They even provided a recipe card. If they wanted to you prepare your favorite recipe, they would have said "MAKE your favorite recipe for everyone to enjoy".

Just the recipe. I had a shower like this and I loved it. My grandma wrote me out an entire recipe book worth (which would have taken a lot of effort from her!) and it is one of my most cherished items, especially now that she has passed. For the gift people usually have something we had registered for that corresponded with their recipe.
This idea is adorable - if I didn't have GBCN authorities on the subject I would have played it safe by bringing my dish in a disposable container with a sticky note I could place / remove if necessary saying that the dish was for the newly weds to save for a night thy don't feel like cooking or whatever.
Just the recipe. This was done for me at my wedding shower. My favorite was recipe I got for a veggie tray; it told me how to put veggies and dip on a tray and serve because I never would have been able to figure that out on my own *-)
