I had it done at my nutritionist's office. Just as a tidbit from my experience: I didn't realize how caged I would feel having my nose pinched shut and only breathing through the mouthpiece.
It gave me a resting rate of 2000 calories. When she read the results after the 10 minutes, her response was "That's a little low for what I was thinking for you". I've severely jacked up my metabolism so she figured it was from me not eating enough for years.
When you put my info in the IIFYM calculator, it shows my BME (resting metabolism rate) as 1960 cal and my TDEE (total daily energy expenditure) as 2683 cal.

No.
BMR is the minimum number of calories your body needs to carry out basic functions like breathing. Any movement at all requires additional calories, which is what the TDEE measures.

Well, more like while sitting in a chair awake as the state you are in while performing the test. But more or less, yes it is meant to be a very baseline measurement.

Yes, body size usually factors into BMR.
Just as an example, look at my first post. I weigh 271. If I weighed 185 (my goal weight), my resting BME would be 1569 and my TDEE would be 2233. So that's a 400 calorie difference in the BME and 450 in the TDEE.

I didn't have to fast. It supposedly is able to read some element of your breath to be able to figure it out.
