First of all, the biting. Second of all the hard sucking on delicate sensitive bits. Third, you know how when your skin is wet and rubbed on for a while, you get a blister or abrasions? That also. Fourth, your breasts fill up quickly with milk sometimes and the swelling can be painful.
Why can't babies burp by themselves? Why do they throw up so much?
You don't really have to burp a lot of breastfed babies but when babies are bottle fed they swallow a lot of air and their systems are too small to handle that much air so they throw up. FWIW SST basically never puked/spit up and BBT only did on occasion. It depends on the baby.

What do people with non-standard work schedules do for childcare if they are working but not rolling in dough?
Most of them hope they married a spouse with standard work hours.
If not, they patch things together. Daycare + grandparents, or whatever they can make work. Or they end up working less or having to change jobs/professions.
How do people who don't have cars in cities like NYC get their babies home after delivering them if they don't have carseats because no car? Do they walk? Push baby in a stroller? Ride subway? Hold it in their arms in a cab and hope for the best?
IME leaving NYU after delivering I still had to have a car seat. We had a car, but most people I know took a cab home if they didn't have a car. Even if you don't own a car or travel frequently in one it's still rare to never ever need to put your child in a car at some point, so most people had seats for when they need them.

Usually either substandard care or they stop working or walk opposite their partner. There are very few options out there.
In PA I used an amazing center while I worked on the campaign that was unlike anything I had ever seen. They offered drop in care for a somewhat affordable rate ($43/day) and we're open from 5:30 am until 11:30 pm so that local shift workers (mostly hospital, some factory) could work. Where I currently live I've found nothing that comes close in terms of hours or flexibility.

You have to have a car seat to leave the hospital, even if you don't have a car. People I know who lived in Manhattan left with a car seat but walked or took a cab home. To walk, they used a stroller frame that holds the car seat. I'm not sure if they installed the car seat in the cabs though or if they hoped for the best since you don't have to use a car seat in a cab.

BIL and SIL are in this situation with my niece. BIL is a newly-trained nurse and has an erratic schedule, with as much as a month's and as little as a week's notice. SIL is a school psychologist. It's a mix of 1) solo parenting, 2) SIL's mom filling in here and there, 3) a babysitter they found on care.com, 4) word-of-mouth referrals for backup care. It sucks and affects so many people with non-standard schedules.

Suction, man. You'd think babies with no teeth can't possibly do damage but they can. Plus the areolas (areaolae?) are similar to your lips to imagine someone pinching your lip realllllllly hard. My pain level wasn't *that* bad, though. Or I have amnesia.
I distinctly remember being at the "I can't do this anymore" point with SST (about 4 weeks in) and cookiemdough talking me down and encouraging me to continue. And I made it through and nursed her for over a year! Thanks again cookie

For some people the pain is a bad latch or, when they're older, the biting. For others it's because letdown is painful. Mine feels like a ton of pressure being forced through my breast.

DS1 was an epic spitter. He'd easily lose 2 oz after each bottle. His esophagus muscle wasn't strong enough to push it down so half his bottle would come back up.
DS2 started projectile vomiting and we discovered he has a dairy sensitivity. So babies can spit up for many different reasons.

For me, the pain was awful for 2-ish weeks. She was a biter when she first latched on. Then it just randomly stopped hurting one day.
She was also a belch-er. At least twice each feeding.

Patience! Lol. I nursed on the uninjured side first (because babies suck harder on the first side, usually) and skipped it altogether when I could (she was also eating solids at the time, so that helped). I covered my poor nipple with neosporin and a bandaid when it wasn't in use. It was torture, and I was almost but not quite ready to wean. I was so sad thinking that our awesome nursing relationship would have to end painfully.

Lanolin, keeping it out to air, rubbing breast milk on it or coconut oil. And trying to nurse with the other side as much as possible.
