A closed pump would be fine to me since a new set of flanges/tubes/etc essentially make it new, and open system pump not so much the risk is tiny but I'd prefer not to take it so it depends on the type.

You're not supposed to b/c of a risk of communicable diseases or something. I used a friends and then passed it on to another friend for my first. Then I got the insurance one for my second kid.

I used a used one (before they were free) and didn't bother me. But - it was older and didn't have the best suction. looking back I would have been better served with a new one.

My insurance only covered a manual pump, fyi. This was only a few months ago that I checked. I believe this varies (whether you can get a decent electric pump) depending on your policy. I have Blue Cross.

If it's a closed system yes. My SIL used mine and I told her to just request using the Medela breastpump at her hosptial which should then give her new tubing and a hand held pump should she need it.

Tell her to look at the Spectra 2 pump, and the ameda hygiene pump kit. It's a hospital grade pump but relatively cheap (about the same as a PISA but with better suction)
Can someone explain the difference between a closed and open system and what the risk is? I have a PISA and am confused about how using it with new tubes/flanges could be a risk to someone else.
Before she invests in buying one, she could also rent a hospital grade one. Through Kaiser it's $43 a month for a Medela Symphony and I like this pump SO MUCH MORE than the $300 Medela Pump in Style I had with my last son. Also, last time I didn't even last 4 months with the pump so that cost was wasted for the most part. I wish I knew to rent one (and a MUCH better pump at that).
3 months of renting would pay for a Spectra S2 which has a higher level of suction then the Symphony.

Open system means there is nothing to prevent milk (a bodily fluid) from getting into the pump and potentially contaminating the BM with things like Hep C. Now the risk is low BUT it is there. A closed system has a barrier system that prevents any milk from leaving the tubes/flanges/etc so if you replace those you are fine.
