I'm a far superior mom who watches her kid like a hawk. This would never happen to us. *flips hair*
Oh wait, yeah it would, lol. She fell of a chair at about 5 months onto a wood floor, forehead first, and a few other incidents. She's STILL falling off things. She was sitting on a bench the other day, horsing around, and flew backwards, landing on her back on cement. :0(

I was holding DS2 in my arms and he was facing forward and rolled out of my arms onto a concrete floor at church. It was awful and I had would wake up in a cold sweat with the feeling of him being in my arms and then not.
H rolled off the futon at something like 2-3 months-- he was laying on A's blanket and when a pulled it H went along for the ride. It was a less than 6" drop on to carpet, but it scared the crap out of me!
B fell off the bed when he was about 4-5 months old. He also fell off the couch sometime, but he landed perfectly on the boppy lol. I left H to watch him while I went into the kitchen. H never even saw him fall. When I went back into the living room I said, "Where's B?". H did not know. I looked down and he was just laying there smiling at me.
W fell out of the swing a few weeks ago. I had only buckled him in on one side (dumb I know) and he somehow wiggled his way out. He didn't cry at all.
My BIL broke his arm when he rolled off the couch onto a carpet his first birthday. My MIL got questioned at the hospital because they didn't believe that injury could be due to such a small fall.
Calvin rolled off the bed a couple times around 4-5 months.
Same for my DS1, right about that point he was such a ninja about it, too. DS2 maybe once? I can't remember, but I was super vigilant by that point.
Oh yeah, and around 6 months my DS1 was in his car capsule which I put on the TABLE (stupid! stupid!) and he tipped backwards onto the floor. Lucky he was strapped in and not hurt.
Yes. Violet has gone off the table at the doctors office and the change table. Owen has fallen from the change table. He was a bit older when he went down a short flight of stairs.
Both my boys have. It was awful. More terrifying though was when E was about a month old, out of sheer exhaustion, I laid him on the floor next to the bassinet instead of in the bassinet. He happened to sleep a longer stretch and I woke up, peeked in the bassinet and saw it was empty. Then saw him on the floor and was sure I had fallen asleep nursing and he had managed to roll offthe bed (the bassinet was right next to our bed). After I woke him up to check his pupils, I realized he had been sound asleep, on his back, fully swaddled and it clicked what had really happened.

Peyton fell off the exam table at the pedi office at her 9m check up. She landed right on the front side of her head. We ended up at the children's hospital and she had to get a CT scan. Which means I had to climb on top of her and hold her down/still. She was totally fine, but it was an awful day to say the least.
