i have always slid a new diaper under the baby before opening the old one and wiping up. For some reason all of our relatives take off the old diaper and leave the naked butt there free to pee or poop everywhere without the protection of another diaper. Whyyyy!?!
I do new diaper under old one, then remove old one. Unless it's going to be really messy, then I don't because I don't want to get the new diaper dirty.
I've done it both ways but for the most part I slide the new diaper under the old one. DD never once peed or pooped during a diaper change so it wasn't really risky I guess??

With a newborn/infant, I usually slide the new diaper underneath. Now, with a 2-year-old, I just hold his ankles in the air, wipe quickly, and slide the new diaper under while his now clean booty is in the air.
I used to put a clean diaper under, especially if I knew it was poop.
Now we risk it - at 2 and some change, he basically holds his butt in the air for me. It's probably a sign that he's ready for PT'ing again, he can practically change his own diaper at this point.
I do new diaper under. My H doesn't. We have not had any pee issues since he was three weeks old. However, in the first three weeks, my H was peed on approximately 137 times.

Naked bottom is exposed for less than a second and no risk of poop or pee getting on fresh diaper.
ETA: I know nothing about boy diapers. lol
Exactly what you said. Except now that I have a boy the open diaper step is open, close, open to avoid pee in the air.
After maybe 2 months I leave the naked butt out there. Never had an issue past the newborn stage. Hell, with newborns even having the diaper under there doesn't even help you sometimes.

Usually new diaper under 90% of the time. I have done it this way since I was 10+ when I changed my sister's and stepbrothers' diapers. My 14 month old is super wiggly and sometimes protests diaper changes, so we've had to get creative.
We cloth diaper and its a replaceable insert if she has only peed so we keep a spare cloth prefold under her butt. It is required about once a week. She has talents.
As a newborn: New diaper under, open old diaper, drop a teeny washcloth on his penis to cover it (we were gifted these and they served no other purpose as far as I could tell), wipe, old wipes into old diaper, secure new diaper and clothing, close up old diaper and toss, pick up baby and head to bathroom to wash hands.
Now: Unfold diaper, set to side, open old diaper, wipe, put wipes in old diaper, close up old diaper and toss, put new diaper on, etc.

I use @this's method. I did clean diaper underneath for a little bit at first, but I kept getting the tabs of the old diaper caught on the new diaper and making a mess So I stopped.
This is leftover from being projectile pooped on multiple times when he was a newborn while changing diapers. I used to change him in the PNP when we had it raised up and had to clean off the sides multiple times. DS had...issues. Sorry for the TMI.
I also keep a stack of those baby washcloths nearby to cover his penis while changing poop diapers to avoid being peed on. Pee diapers I just slip the old one off over top and have the new one ready to go underneath.
I don't think I have ever slid the diaper underneath first. Seems you risk getting that diaper dirty if the old one is poopy.
We use cloth so there is always a prefold underneath though.
For DS we did do open, close, open more than with DD.
This was my same exact thought! And I thought it might be unique to us cloth diaperers No way would I risk dirtying a CD in 2 seconds. I'd rather them pee on the changing table lol.

I tried putting a new diaper a few times, but am clearly doing something wrong - the tabs on the dirty dkaper would stick to the new one, and then I couldn't pull them apart and just had a yuck wad of diapers. Much easier (for me) to just have the new one ready, open the old, clean bottom, wrap up wipes in diaper, then put on fresh.
Last night we tried a standing diaper change. Lol forever.
