I don't record any time off, other than a placeholder .1 billed to "time off" if i am actually out the entire day and have nothing billable because time recorder rules require you to input at least .1 for every regular workday.
I don't really know. If anyone on my team takes less than a half day, they just make it up later. Or they don't. I don't really know or care as long as things get done. I sure as hell don't take bits and pieces of PTO since I usually work 6 days a week.
I have no idea. I've always made up the time if I was gone for less than a couple of hours. I pretty much always work more than my required hours anyway so this isn't usually something I even consciously make up.
Most of my supervisors didn't want us to bother if it was under an hour, but our system does allow for 1/2 hour increments and I did have one supervisor who insisted upon it.
Unless we are gone a full 1/2 day, it doesn't count against us. I typically work through lunch or just run into the break room so they are pretty chill about me missing a couple hours here & there.
We only report in half and full days. Can you work through lunch another day this week to make it up?

No job I ever had in the U.S. was this fussy about stuff like this. I could always either make up the time specifically or it just ended up being a wash overall for the times I had to stay late to work on an urgent deadline.

Ok scratch that. What I mean by we is "those that report to my nitpicky manager"....every other manager in this building does not micro manage.
