I'd be interested to find out, so I would ask the waiter/staff. But I would not assume it would be like falafel, because hummus is finely ground chickpeas, almost like a paste, while the falafel I know is a coarser ground, more like grits. I would think it would have to be a patty of some kind though. My only other suggestion would be similar to baked brie, where the hummus would be wrapped in phyllo or some other form of pastry, then use as a dip/spread.

I've never heard of it, either. My guess would also be that it's some kind of patty, although in the hummus sandwiches I've had, they just spread the hummus on the bread and then put the other stuff on.
