Right now all of DD's friends are the children of my friends so they call me by my first name. The kids in the neighborhood that come over to play with DD, but are actually much older mostly call me Mrs. Lastname because I've worked in their school for years. I don't really have a preference and just encourage them to call me what their parents tell them to use. I don't want to go against their parents' wishes if their parents want them to be more formal.

Ms. Joe at my home. At one of the kid's schools, the norm is Ms. Bunny. At the other, it is first name, Joe, no Ms/Mr. even for teachers. I am most comfortable with Ms. Joe.
I live in the South. Miss FirstName or Doctor FirstName. It is also how Caramini addresses our friends or other non family adults in her life.
You're a dentist, right? This reminded me of a funny story. We were taking Jack to the dentist for the first time when he turned 3 and we kept explaining that Dr. (John) Smith was going to be his dentist. We talked about it for weeks, so that he'd be more relaxed. He even had big plans to ask Dr. Smith if he could drink milk after he brushed his teeth because we told him he couldn't. When Dr. Smith came into the room he said, "Hi! I'm Dr. John!" Jack immediately responded with, "My dentist is Dr. Smith!!" It took a while to smooth that one over with Jack as he was insistent that this imposter go get his real dentist.
To be fair, at work, I call myself Doctor LastInitial with kids. But I am a specialist and don't see a ton of kids in my line of work.
I hope that your son and his dentist were able to regroup. LOL. And also I hope that he was disappointed about the milk.
My best friend's kids call me "Aunt Skib" and other friends of my DD call me either "Miss First Name" or just use my first name. No one calls me Mrs. Last Name at this point, but I expect that will change as DD gets older and her friends are outside of my social circle.
My kids call everyone first name unless someone asks them to do so differently. I teach at a Quaker school and they go there, and all the teachers go by first name only. They call virtually no one "Mrs" and "Mr".

They usually try to call me mrs. Last name and I correct them and ask to be called by my first name only. . I don't like being called mrs. Last name and prefer to go by just my first name.
Usually Mrs. Last name. I refer to myself by first name, so I'd be fine with that as well. My kids call their coaches Mr/Ms First name, but friends parents are always Mr/Ms last name.
