like this?
We have one very conspicuous vent that used to be next to a wall, but since we renovated it's basically in the middle of a walkway. Our lovely contractors left the metal vent cover on it and I can't count the number of times that my kids have snagged their socks on it. Now I'm looking into vents like these as an option, but wanted to see how it works when you install them after the fact. I'm not super concerned about the stains not being a perfect match since the wood tones in our floors kind of vary anyway. I just really want it to be flush with the rest of the floor.
I'll have a carpenter or a handyman do this, any BTDT advice?

We have those and I'm pretty sure you can buy them at Home Depot, Lowes or any flooring store. Just search 'Flush wood floor vent'. Its a super easy DIY. You just pop them in.
We had flush vents installed when they put in our hardwoods, and I love them. I don't know what would be involved to do them after the fact, but I would consider also talking to a hardwood flooring person about doing it for you, since they do these all the time.
