My hospital gave me the 5/5/5 rule: 5 hours on the counter, 5 days in the fridge, 5 months in the freezer. My understanding is this is a very conservative rule. I followed 5 hours for both refrigerated/warmed and freshly pumped milk.
ETA: Scratch that, after warming it, I think I usually tried to get it back in the fridge within a couple hours. But if it doesn't smell bad, it probably isn't.

If it's frozen, we warm it up with hot/warm water, and she only takes a bit from the bottle, can we put the bottle back in the fridge?

Yep, I would just use the bottle at the next feeding and toss anything left over after that for your first scenario.
For the second, also yes, just use the rest within 2 or 3 days (is what I did... don't know the "official" recommendation on this).
