My job sent us an updated insurance information booklet. It mentioned a few name brand IUDs that are completely covered (other than your copay). I'm not sure if that part is specific to my insurance company....
H's insurance sent a similar notice. Considering the fact that the cover almost nothing, I assumed it must have been required. Ours goes into effect jan 1, but I don't think that means anything.

There may be some exceptions for grandfathered plans. I'm having trouble finding exactly which plans are grandfathered, but if yours is, policy documents much state it.
I think they are. I had one scheduled after the new laws went into effect. Instead of paying $100 outpatient cost as expected, I only had the $20 copay.

They are. UNLESS your plan is "grandfathered in" (which mine is.).
I've been told that most grandfathered plans will expire by 2014. Your HR person should be able to tell you if you are a) granfathered and b) when your policy renews/becomes ungrandfathered.
