I had an appt with a OB like twice and the second time kind of annoyed me since he said something about weight gain. I was doing perfectly fine and none of the MWs had said anything.
After that, I made all of my appts with MWs. I wanted a med-free birth so I felt they could advise me better on that. The default in the practice is for the MW to deliver the baby unless there is something wrong or out of the ordinary. I had a normal pregnancy and delivery so I never saw an OB until we did the circumcision.
This pregnancy, I'm going to a birth center with only MWs.
Mine was. I started out as a midwife patient, but when things went south with E I was automatically transferred to a doc. I loved them all and really didn't notice a difference between the 2 types - they were all hands on and all very open to any birthing plans (within reason obviously) and all very patient and caring.
I did, though it sounds like mine was a little more delineated than yours. You would basically choose whether you were going to be a patient of the midwife practice (5 CNMs) or the OB/GYN practice (8 drs). They shared the office and had a lab and U/S machine so that everything was done in house. It was attractive to me (I moved to them at the start of my 2nd preg) b/c I had a history of PIH and pre-e with my 1st preg. I preferred to go to midwives, but wanted to make sure that if something went wrong and I had to switch to an OB practice that I wouldn't have to start from scratch.
