Now, if we're getting out of the car, I open my own door. But he opens my car door when getting in and to restaurants, etc. I like it.

If we are approaching the car, yes. I don't sit in the car and wait for him to come around and open it.
His car has manual locks so I think it would be weirder if he left me standing in the cold while he went to his door, fiddled with the keys, got it open, got inside, and reached over to unlock. I appreciate it.

Sometimes. I won't be like, waiting in the car for him to open it, but sometimes when we are getting in the car he will open it for me. When we were dating he had a truck that he had to manually lock and unlock, and he always unlocked my door first and opened it for me rather than unlocking himself and scooting over to unlock the door from the inside. I thought it was sweet.

He always opens regular doors, though. And if I go into a store alone and come out carrying something, he always jumps out to open the trunk for me.

Yeah, I think H opening my car door is more about him and less about me. I would feel like a total bitch if I was all "I can get my own car door, goddammit!!!!"

(the high maintenance thing obviously really bugged me!)
