We usually go pick it up because we rarely carry cash or don't want to wait (most places near us take at least 30-60 minutes to deliver, sometimes we've waited almost two hours)
I tip 20% anywhere you sit down, whether or not it is a buffet (although the only buffets I've gone to in the past 10 years have been at fancy hotels). I know in theory that you can tip on the amount before tax, but I've never done that.
I tend to tip $1 to $3 per drink, depending on how drunk I am.
I usually do not leave a tip when I pick up food for take out, although I feel like an ass since there is a tip line when paying with credit card.

At bar- Do you tip $1/drink, 20%, other? If I'm buying one drink I leave $1. If I'm running a tab I tip the usual 18-20%, again, rounding up. (I only drink soft drinks though; if I were ordering a fancy $8 drink I might think more than $1 was expected.)

Delivery? I don't add a tip to the bill when I'm paying but I give the delivery person about $5 to (maybe the highest ever) $15. This isn't based on a percentage of the bill though; it's more based on how unpleasant it was to deliver my food. A rainy night I know they're busy was the $15. The $5 tip was for one pizza he brought to my kid when I wasn't home. I just told my son to give him a $5 bill.
You didn't ask, but my idea of a good restaurant experience is a great meal and a bill that, with tax and tip, is divisible by 10. I love when I can add in an odd-amount tip and have the bill come out to, like, $60 exactly.
It's wacky. I tried to pay for a cab ride last week, and the driving pushed my change into my hand as I was getting out of the cab (I figured the change would make a small tip - any cab I've taken elsewhere would have just kept it).
There's no "tip" line if you use credit cards to pay at restaurants.
Back in the US, I tip around 20% for most things. Canada I tend to go a bit below that. Most of Europe I aim for about 10%.
