I gave up on using a duvet cover today. I'm just so sick of my comforter sliding all over the place when I make the bed or sleep. Even with the little clips, it just wasn't cutting it; it still came unhooked and slid all over the place. I love the convenience of being able to easily wash the duvet cover, but having my comforter start out by my chest and slide down inside the cover by the morning was so obnoxious. So I gave up and bought a comforter set today. Best part? $40 on clearance from BB&B. DH was quite happy with my purchase as the duvet was driving him nuts, too.
I sewed grosgrain ribbon loops to the corners of my duvet, and ribbon ties to the corners of my duvet cover. That holds it in place relatively well. I have thought about adding a tie to the middle of the head end too, but haven't had enough trouble to justify the effort yet.
This has always baffled me. I've done duvets since college and they've never slid around on me. I'm just mystified on how it can happen for so many people but not me on any combo I've used over the years.
No a fan either. Had one duvet from Restoration Hardware and never really like it, couldn't get it to ever look quite right even with the clips. Big comforter fan here!

Same here. We aren't currently using our duvet, but we did for many years and never had issues with it.
ETA: We've never used clips or ties. I do stay in pretty much the same position all night, but DH is kind of a crazy sleeper.
Our newest duvet cover has 8 ties. One in each corner and one in the middle of each side plus the top and the bottom. I had to sew new ties on our duvet since it only had ties in each corner but that sucker ain't going nowhere!
Haha, indeed. Maybe DH and I are just crazy sleepers and make the comforter come unhitched from the clips or something, but we just can't make it work. It'll be nice tonight not to have to wake up half way through the night and throw on an extra blanket to cover the spot where the comforter has slid around the duvet cover!
I use four pins in the corners and stitched velcro tape down the center of the duvet and the comforter. We are using DH's very warm and very ugly comforter from his bachelor days as an insert so I didn't care about the look of the insert. Stitching the velcro tape by hand on the comforter and duvet was a total pain but the results are worth it.

Ditto this exactly. Maybe because I buy cheap duvets so the cloth isn't slippy? The covers are a wide range of quality though.
