We had this done a few weeks ago. I can tell you, I thought it was a joke. It might not be the same in your area, but ours is called Child Outreach, and they want all kids between 3 - 5 screened.
The basically sat her down and said here are 5 blocks, make a tall tower. Here are three blocks I will stack, stack them like me. Then again differently. Here are pictures, what are they - a tree, sneaker, wheel, button, etc Draw a person, draw something else that was super easy Can you spell/write your name Can you stand on one foot for 5 seconds Can you walk in a straight line I am going to give you directions on how to make a PBJ sandwich, you give me directions on how to brush your teeth. Then again with I think putting shoes on. Then they did an eye test.
They said "wow, your child is really bright and excels, we'll send a report in the mail."
I have heard the report is worse than the screening itself.
ah, i see. i'm sure i'll remember to sign my kid up for it. just like i did for the next set of swim lessons, except that i didn't and then had to beg the aquatics director at the Y to let her into a class (which she did, because she is awesome).
We apparently don't like to early screen in Iowa. Or I missed it. I am going to register Jack for K tonight though. I hope he doesn't get stuck in the dumb class for kids who don't read so good.

Now I am all nervous because he can write his name but won't do it on a straight line. he is not the best with a pencil. He is such a fucking hipster, always trying to be ironic.
