The office should be able to give them to you, shouldn't they? The last time I had to provide lab codes to my insurance, their billing office had them.
I think I've done this. Call the lab that you go to (I use labcorp, for example) and ask them for the code. They should be able to tell you what it is. You could also try the company that processes the test and ask them. I had to get genetic testing done and I called the company that did the actual testing and they knew the codes.

yes, the hemo nurse could tell me but she's out of the office until tomorrow (i tried calling a little while ago) and i was just trying to shoot an email out to the dolt that is fucking worthless at bcbs so that she can "work on it" first thing tomorrow morning.
i have been dealing with this dumb shit for a month and just want my kid to get his fucking blood drawn so that we can adjust his meds without it costing me $300 oop.
I was hoping to use one of the sites we use at work but I can't find anything that doesn't require a login/password and I don't have that at home. Sorry
What's the test? I just googled j code or procedure code for thyroid test and found it.
i did this, too, but all i could find was something that was specific to an outpatient lab (all numeric and shit) and i got all frustrated and decided to post here.
