Who do you use as references when you don't want your boss to know you're looking for a new job? I know my old principal would do it (which is great because she went there and both her kids attended that school!), and I asked a coworker and she would, too. Who else would you use?

PDF, shows that you actually know how to create a pdf and the format is the same on all pdf readers.

What industry are you in? I use a functional resume, I have had the same job since college but several positions and a lot of skills so a chronological resume doesn't look great for me.

I list references available upon request, the two that you listed are good. Have you done any tutoring for anyone? Any other administrators, dept heads, etc that would speak for you?
1. If they don't have a prerequisite, then as a pdf. Formatting doesn't change. 2. Depends on the job/industry. Google your position/industry, and google an example. 3. Colleagues, people I've worked with at different boards, volunteering directors?
