I have, especially when the order is large or complicated and the server goes the extra mile in making sure everything is in place. It still takes work to pull all that shit together.
Yes, we get take-out sushi regularly for lunch and we have several orders and everyone wants different sauces, etc. so I always tip the gals for their trouble. They have never gotten my order wrong.

Take out servers typically make more than regular servers hourly. So if a reg server made $4.25 an hour than take out made $6.25 an hour. At least that's how they did it where I worked in college.
I do, but it is less than if I was sitting down at the restaurant and being waited on.
I didn't until I waited tables in college and we had to do the take out meals too. I got paid 2.83 an hour and the take out meals still had to be claimed as part of my sales that I had to claim tips on.
When I do take out, it is neither complicated nor large. And it's usually exclusively at take out places with counters and set-ups for easy in/out. So, no tip.
Subway and pizza type joints, no. Chinese at a sit down restaurant, yes. Most times we just get it delivered to our house anyways and then I tip for sure, lol.
