It's certainly different than the standard onesies and gift cards. I'm an organizational freak and would love something like this. I strongly recommend you include a gift receipt, just in case. Kid gifts with no gift receipt are the bane of my existence.
I lean toward no, but you know the mom best. It's essentially just a file folder she could get at Office Max for $10 but with cutesy colors and decals. If I wanted something like that, I think I'd just go pick up a plain one.
It's okay. I have the same concept with a file folder from the Dollar Spot at Target though. I can definitely list off things that are more useful for the money for sure.
It seems sort of pointless. Important documents, like birth certificates, SS cards and passport go in the family safe deposit box. His pedi gave him a booklet for tracking well visits and vaccines. He didn't come with a warranty and most gear that does isn't something you'd hold onto long term.
Diapers, as pp mentioned Basket with stuff like gas drops, Tylenol, thermometer, first-aid type things I love the aiden+anais blankets but I know that's a popular gift - same for Sophie the giraffe The fisher price ocean wonders seahorse is magical Toys are great too, as are books.

Every single one of these were things we used for Gabe. We actually have 2 seahorses because he has one at dc and one at home.
I actually think the concept is a good one. I created my own "emergency" binder of information about the kids in case we ever have to rush to the ER or if we need to get out of the house quickly. It has all well visit reports, vax records, therapy records, dental records, school health check records, etc.. I also keep copies of their birth certificates and SS cards for quick reference.
FYI, I would not get diapers and instead would give a gift card if you are going to go that route. Some diapers don't work well for kids because of the fit, and you are better off letting the parents sort that out and get what they know works for them.
Please don't give blankets or stuffed animals as they will get a thousand of these.
Personally, I always give books because you can't go wrong with them and while they might not be immediately useful, they always eventually become useful. They also speak to my personal and very strong interest in education, so it feels more like a meaningful gift.

That's why you give them with a gift receipt and Walmart will exchange all diapers as long as they aren't opened without a receipt.
I wouldn't get diapers either, but we CD, so it would have been pointless.
Books, A&A blankets, I love my extra big "boutique" sized one that is more expensive, and I only have one for each kid, that would be useful. DS was/is really long, so the bigger blanket made swaddling easier.
The gift basket with gas drops, etc would be good. Is she planning on nursing? A little basket of things to keep by where she'll nurse would be good too, a book, an iTunes gift card, granola bars, water bottles. A nice reusable water bottle with some stickers to decorate it with?
The other thought is to go a different route, you don't have to get something for the baby. Put together a basket with a magazine, starbucks card, chocolates, finger nail polish, chocolates, and a gift certificate for a pedicure. She will be thrilled to have something that is all hers and will remember it one day when she's have a really bad morning and it will make a huge difference.

I always do a basket (like a diaper caddy) with the essentials. Butt Paste, Tylenol, teething tablets, gripe water, nail clippers etc. No one wants to buy that stuff, so I do it.
Wal-Mart will exchange unopened packages of diapers, so I never worry about that, but I always check to see what brand they registered for and if they didn't, I skip diapers.

This, exactly. Except I didn't get any a&a swaddles. But I bought myself 6or 7 of them because they are that amazing.
