I'm sorry you're not feeling well. That sounds like a really rough thing to deal with all the time.
Ask about your ears and sinuses. Sometimes vertigo can be caused by an ear problem.
Ask what the plan of action is next. Make sure there is a plan in place before you leave.
Have you had your eyes checked recently?
For the most part, the doctor should be able to give you a plan and a timeline for what steps will be taken and possibly what it could be. Make sure he knows how serious it is and is taking it seriously.

A girl at work's daughter was always tired and "vitamin d deficient" but the vitamins weren't helping. Ended up her mono had come back but the dr didn't think to test for it. I'm no dr, but it is worth a shot!
I agree with the ears as well. I have had dizziness/vertigo for years because I had stopped taking an anti depressant cold turkey before they knew it caused side effects. I feel your pain in terms of the dizzy spells. They suck. Definitely check on mono as well. Good luck,!!!!
I would see an ENT about your ears, too. Something going on with your inner ear can cause vertigo. I hope they figure it out soon...that has to be so annoying and debilitating.
Another vote for asking about ears. An immediate family member of mine has Menieres Disease. It began in her 30s with completely debilitating dizzy spells. It's now completely under control with meds.
