The actual removal and replacement is easy. The only concerns are if it's against a wall and what the flooring is like underneath if you change the footprint.
For example, in our hall bath it was evident that there wasn't tile under the vanity b/c instead of replacing the tile when they put in a pedestal sink, they just put in other beige tile to fill the gap.

I'd say yes, it is about medium difficulty in the DIY range. As the pp said, if you don't have to touch the flooring, than it's just a little plumbing change-out and some securing to the wall. If you have to redo flooring than it gets more complicated, but not impossible if you have tile.

I would say it's about moderate difficulty. I would not do it as my first DIY project, but my H and I have replace 2 vanities on our own. Check out some youtube videos first to see if it's something you think you can do.
Also, we didn't have any problems with the floor, but it's a good thing to keep in mind.
I managed to replace a vanity at our house by myself. It was not that difficult because the old vanity was obviously sitting on top of the tile (and was almost falling apart).
