I always recommend, and will always use, Travel Guard. I had to file a claim with them a few years ago literally while we were sitting in the airport getting ready to leave for our trip and they were amazing about reimbursing us and making the whole process seamless.
Others around here typically recommend insuremytrip.com for comparison shopping, if you want to shop around.
I think we have the Norwegian version of TravelGuard through work (since it's handled via AIG/Chartis). I'm not sure how the US service compares, but I love it. They're very prompt at repaying my claims, and I've never had a reasonable or borderline claim rejected.
My past claims have been: - Foreign emergency room service and pain meds after a serious back pain flare-up - Foreign urologist appointment for a UTI - Delayed-luggage reimbursement for clothes and toiletries - The cost difference after the amount Air France paid to replace my broken suitcase (I filed a claim for the extra $20 I paid to get a blue suitcase instead of a plain black one. Since I was replacing a blue suitcase after all!)
