Yeah, this is kind of my job except we investigate claims that were overpaid or incorrectly billed by the insurance company and recoup from the provider not the patient.
There may be a certain amount of time for them to correct the claim. I used to work at an insurance company and we had 6 months from the TOS to adjust. After 6 months, the insurance company had to pay and couldn't charge the consumer. You can always call and ask your insurance company.

I kind of feel like insurance companies just do whatever they want because most people dont know what's allowed and what's not. So...I have no idea, but I'd appeal it if it happened.
