Not scary, but the most DISTURBING movie I've seen is The Cook, The Thief, His Wife and Her Lover.
This is one of my all-time favorite movies. Some scenes are hard to watch -- but it's so well done and beautifully shot. I really love the Thatcher parallel. I thought it was really fine satire.
I mainly couldn't handle the guy getting book pages crammed down his throat, but I did enjoy seeing a young Tim Roth.

Oh, god, yes. The first was awful, the second was wonderful. Michael Gambon was absolutely despicable. It's weird to watch that movie now and be like, "...Dumbledore?? You were MEAN!"
I love paranormal movies, serial killer movies and monster movies. Not slashers though. I thought Hostel was disgusting.
Movies that have actually scared me:
Fire in the Sky- This movie scared me so bad as a kid. Xfiles - The Movie - Love it The Ring - Terrified me Paranormal Activity - I hate the sequels, but the original was brilliant The Possession - This movie is SO good and well done The Exorcism of Emily Rose - This movie legit freaks me out and makes me unable to sleep It - One of my favorites The Conjuring - Very well done
Ones that I love but didn't really scare me much:
Insidious and Insidious Chapter 2 - Both great movies The Evil Dead series - Old favorites VHS - Really weird movie, but super interesting and creepy The Host - This is a fun monster movie. The Bay - A bit slow, but the idea behind it freaks me out. Troll Hunter - Super interesting plot Resident Evil (original) - Fun, scary movie Don't be Afraid of the Dark - I really loved this movie, it was fun and creepy
One that really scares me:
Evil Things - The movie itself isn't the most scariest ever, but the idea/plot are frightening. The stalker just randomly targeting groups/individual people to stalk and kidnap/do whatever to, but its the extras that really scared me that are all tied to the main movie. one is where the person/whatever sends an anonymous video to the FBI. It features a young woman that had been missing for awhile. The label on the video said "Stalking 101" and it just shows someone filming the young woman as she goes about her day. Super boring stuff--cafe, errands, etc. The person just constantly films her and follows her around without her knowing. Just thinking about it is so creepy and scary.
I guess it has to be The Blair Witch Project just because on many occasions when they are in the tent and something scratches it and they run off ran in my mind. I think it kept me up at night too.
Does anyone remember those movies where some people were stranded on an island and they discovered giant rats or ants? My dad use to watch them in the afternoon when I came home from kindergarten and they freaked me out.
The Ring and Grudge, haven't seen anything after the Grudge.....but the Ring seriously messed me up, I still don't think I'm over that one....H and I went to see it (the Ring) when it first came out, all high school kids in the theater (except for us) and pretty much every girl there screamed their lungs out, it was horrific and yet funny all at once.

Top 5 for me
1. The Strangers (could not finish) 2. Event Horizon (had to go home to an empty apartment) 3. The Ring (roommate and I freaked the fuck out when - a few hours after we watched this - the TV turned on suddenly without any obvious input from us. Turns out he sat on the remote control, but holy shit we were ready to move far far away.) 4. The Descent (probably my favorite horror movie next to Scream) 5. Hostel (nononono. This movie is the reason I can no longer watch gore / slasher movies.)

Also, I thought Funny Games was awesome. I tend to go for the more disturbing movies than scary (Battle Royale, Audition, Irreversible, I Spit on Your Grave, etc.) Probably doesn't bode well for me as a person.
