I posted this same thing on April '12 but then I thought maybe frkls might know something about this after her boob course or anyone else who might have a clue?
I have both thrush and mastitis and my midwife prescribed Diflucan and something else (didn't catch the name, it was called in). Pharmacist subbed in erythromycin, then wouldn't fill the Diflucan because of interaction. I was not impressed with her so I'm wondering if that's true (why would my midwife prescribe things that could give me heart problems) but mostly I just want to feel better.
Can both mastitis and thrush be treated at the same time? I've had thrush on and off for a month and a half and I'm miserable. We've done three rounds of Nystatin for both the baby and me. I've sterilized everything and somehow it keeps coming back. Can I do anything to treat me while on the drugs for mastitis? It's really painful to feed him (both infections in the same breast). So I would think they CAN be used together but it has the potential for side effects. Maybe ask the midwife for clarification. Also, this is probably so wrong, but I trust pharmacists more than doctors when it comes to medication side-effects and interactions. And I am a big rah-rah doctors person in general.
Thanks I generally do as well, but this one was not confidence-inspiring. I'm glad she caught it if there's an issue but I didn't enjoy being scared or the hour wait to pick up one thing of pills because she had to keep calling people (my doctor, the main pharmacist who wasn't in).
i'm so confused. the pharmacist subbed in a medicine that can't be taken with diflucan and then refused to give you the diflucan? why didn't the pharmacist just fill the RX the midwife sent over? i'm obviously missing something.
have you tried gentian violet? might be worth a google
She subbed in something that can't be taken with Diflucan so she wouldn't fill the Diflucan prescription. I have no idea what my midwife originally prescribed since she called it in so am not sure if she knew there was an interaction. The pharmacist was really strange (nice, but strange) so I was skeptical if she was correct. Apparently she is, so no Diflucan for me I have bad thrush on the same breast I have mastitis (red, peeling, cracked nipple) so I was really hoping I could take both.
I had a friend with thrush and she swore by doing a vinegar nipple wash (in addition to medication she was prescribed). From the LLL website - Rinsing your nipples with a vinegar and water solution (1 tablespoon vinegar to 1 cup water) after every feeding is helpful. Use a fresh cotton ball for each application and mix a new solution every day.

The pharmacist can't just substitute what the midwife prescribed. She could give you a generic, which is equivalent to the brand. Or she could call the prescriber for a different antibiotic in case of allergy, interaction, etc. Also lots of prescribers are unaware of potential side-effects and drug interactions. It isn't a huge part of their education.
I hope you get rid of both problems soon. BFing is hard sometimes!

If you are desperate I would use Grapefruit seed extract to kill the thrush. I once had to wait a week until I could get in with a new OB and drinking it mixed with water stopped the horrible, shooting pain and nipple sensitivity. If you google grapefruit seed extract thrush you will find some dosage info. It tastes disgusting but it's worth a try. Hopefully your midwife will know of any harmful interactions. Good luck.
