A couple of weeks ago, I ordered a wet bag from a shop on Etsy to use as Andrew's toiletry bag. I got the item quickly, no problem. I went to Hilton Head for a few days and when I came home, there was another package from the seller. It had another wet bag in it. The packing slip had the same order number. I checked my order and paypal and only got charged for one. I e-mailed the seller and told her that she sent me an additional one by accident. She sent me a shipping slip so I could send it back. Cool. No problem. Well, I just got an e-mail from paypal saying that she refunded the cost of the wet bag, minus shipping. I'm guessing she thought I was returning the product I ordered? I even included the original order and a note.
Do I contact her and let her know that she refunded money that she didn't need to? Do I just keep it and move on? Obviously, I know what the right thing to do is. I just get extraordinarily frustrated when people who run small businesses are completely inept. Fulfilling my order twice is a mistake that could happen to anybody. But refunding my money when I included a note that said I was returning the extra bag she sent me? Ugh.

Something similar happened to me. I contacted the person to alert them of a mistake because I didn't want weird karma and it would have weighed on my mind forever. I just sent a payment to their Paypal account.

She refunded $17, the cost of the wet bag. The original order was for $21.25 I think, which was the bag plus shipping and handling.
At this point I would just move on. She is obviously confused and/or unorganized and you already did the right thing once. I don't think it should be on you to spend time and effort to help correct a mistake she made a second time.
Maybe she just did it to be nice, to keep her reputation good or something? I would feel awkward too, but would probably take it as a nice gesture and woudnt want to deal with it anymore.
I would email her and let her know, step by step what had happened. It could be she has someone working for her and she doesn't realize the person is incompetent. If she's trusting this person, she might not realize it unless customers tell her there's a problem.
I think it would weigh on my conscience, so I'd repay her via PayPal with a note, like others suggested. If she screws it up again my conscience can eff itself.
