Our shower was a surprise. I had nice flower arrangements delivered to the hostesses (my MIL and SIL) a few days later. I think they were about $50-60 each.
My hostess had her own baby shower about a month after mine (I had a baby by then, so didn't host it). I bought her some very nice, high-end items from her registry.

I feel like the best gift you can get someone is something they will use or appreciate.
My sister is hosting one of my shower's so I am going to get her a Peet's gift card since she loves coffee and an additional item that I haven't thought of yet.
My best friend is hosting my other shower and I know she likes wine so I might get her a nice bottle of wine.
Philosophy shower stuff is always nice because most people don't buy it for themselves. That is my go-to gift for friends for birthdays!
Things I've gotten in the past that I've liked (but can't reuse since same friends will be hosting) are this necklace (http://www.amazon.com/gp/aw/d/B00A6FEGMM) and monogrammed stationary.
I gave my friend a Sephora gift card, makeup case for travel, an inflatable travel pillow, and cupcakes since she told me that she is really looking forward to travelling this year.

My friend was the "main" hostess & I got her a groupon for a massage (she won't think I'm cheap for it..), a gc for a pedicure at our place & gc for Starbucks... All come with an offer to babysit.
Her mom helped her a TON so I got her a sephora/bare minerals gift pack. Another friend did some day of help and I gave her a little soap/lotion thing from homegoods. Oh and I made all of them & the grandmas some macarons that I packaged up.
