Questions to ask in-home daycare provider

We are meeting with an in-home day care provider tomorrow. We are 99% sure we will use her based on two recommendations I've gotten from colleagues at work, so this is really more of a "meet and greet" with time to ask questions.

Here's what I've come up with so far:

-monthly charge
-hours available
-max number of kids she has in-home
-what does she provide? What do we provide?
-her closing calendar/days off/etc

What else? Help!

I also asked about policy on medication, both OTC and prescription. Most will give it if you provide it with directions but I've heard some won't give prescription stuff.
