Not until after a year. I guess when he moved to a cot at daycare so around 15 months. DS wore a sleep sack with footed cotten PJs. I don't see a need for a blanket prior to a year (absent use as a lovey). The sleep sack does the trick
ETA: I will add that he used the blanket only for naps for a LONG time. I think closer to 2 years he started to ask for a blanket at bedtime and then at around 26 months or so he started to refuse the sleep sack.

We put one in the crib with DD when she was about 5months. They're muslin, so not thick, and we kind of put it on her like a toga, with one loose end over her shoulder (just enough that she can suck on it, she never took a paci), and the rest tucked around her in a loose swaddle. She doesn't move much when she sleeps, and she doesn't roll yet at 8 months.
DS is 17 months. He still doesn't have one for overnight sleep, but I'm certainly not opposed to it. We just still use sleep sacks. For naps at home and at daycare, he gets a blanket. We've never put anything under his PJs. When he was tiny and swaddled, we actually often had him sleeping in just a diaper or just a onesie. I didn't want him overheating.
My kid hated swaddles and Sleepsacks. She has slept in just a cotton sleeper since about 6 weeks old. On super cold nights we used fleece sleepers. I just offered a blanket this week and she promptly picked it up and worked to drop it to the floor. I'll try again next month, lol.
DD is just shy of 10m and has had blankets in the crib since...7m or so? She was pulling up and crawling, and is now walking. She sucks on the blanket to soothe herself. We also used a smaller one like a pillow when transitioning her from the swing.

DD has this www.littlegiraffe.com/shop/baby-blankets/security-blankies/luxe/luxe-dot-blanky/ and I used to only let her have it for naps, starting around 4 months or so?? But as of a couple of weeks ago, I let her sleep with it at night; she's been rolling both ways for a long while, and has been sleeping on her tummy almost a month.
fwiw, the pedi totally gave it the thumbs up at her well-care this past week when I told him. She rubs her face on it, bites it, drapes it over her head, basically it just makes her feel secure, which is the point of a security blankie, right??
Under her sleepsack, she wears the footed jammies from baby gap, mainly the zippered ones, and when it was really cold, she wore a onesie underneath that too. Sometimes socks even. It would get down to like 60 in her room.

Not yet (8 months). We will let her have her little A&A blankie/lovey soon but for now she just sleeps in fleece jambes and a Halo sleep sack. We won't give her a full blanket until closer to a year.
DS is 2 years old, and still has no blanket. He sleeps in a footed sleeper and a sleep sack. Our pedi said he prefers the crib to be "solitary confinement", no blankets or pillows b/c of the suffocation risk.
We did a loosely knit blanket starting around 6 or 7 months. The knit had pretty large gaps in it, so I wasn't worried about suffocation if DS pulled it over his head.
Until then, my MIL knit DS an infant cocoon pod that we used every night (example below from the internets, not my kid). DS's bedroom is usually pretty cool, so not having some kind of blanket wasn't an option and he HATED sleep sacks.
blanket around 13 months. My grandmom knitted him one, I figure its light and the holes are big enough to not cause any suffocation.
He wears cotton footed pjs (or fleece for really cold nights) and fleece sleep sack over it. We put the blanket up to his armpits or mid back if he flips right away. The blanket is never on him in the morning, ha.
