The mother to be has a registry at Walmart but she is young and it's kind of random so I am trying to figure out what to give her. They are having a boy in October maybe earlier she has been on bed rest since 18 weeks. I bought material to make a nursing cover but I don't feel thats enough.

When I don't what to buy or think the registry is ridiculous (Yes, flame me, but I've though that many times) I go with something basic, simple and useful. Childprofing kit, sippy cups for when they're older, a Target card for diapers or whatever they need, baby soap etc. Books and pjs are cute and my fave to give. Makes a fun little basket and I know while Rubes more clothes than she'll ever need we always seem to be short on Pjs (always in the wash, naturally fit tight, so she outgrows them fast etc).
I tend not to go with registry items for baby gifts. I recently got my GF her diaper covers from her registry and I ended up spending more than I had really wanted to.
Anyway, I like to get a book, a plain set of onesies, and then one other thing. Sometimes a toy, sometimes one of those hooded bath towels, sometimes a gift card. I know a lot of people like to get outfits, but most of my friend's kids live in onesies 99% of the time for the first few months anyway. I also usually get the next size up. A lot of people forget that the kid will need clothes even after newborn-dom.
One of the best gifts I got was a hooded towel and blanket set by Swankie Blankie. It was useful and functional like the essentials I had on my baby registry but also so luxe and pretty. Not the kind of thing I would have splurged on myself, but it was one of my favorite gifts. swankieblankie.com/category_1/Security-Blankets.htm Mind you, if you know how to sew, I'm sure you could make one of these pretty easily.

Do you know for sure if this person is planning on BFing? Also if she goes too early (which sounds possible since she's been on bed rest since 18w) her milk might not come in and she might not have the chance to BF. I think you definitely want to have something that you can give and then include the cover if she makes it far enough. One of my friends had a pump on her registry but ended up going early at 30 weeks. She never got her milk but her aunt got her the pump (before she knew she was going early) and now it sits on a shelf and makes her sad because of the things that she wasn't able to do.
- book - basket of natural baby products (this can be expensive and something new young moms may not spluge on) - stuff for when baby is older. Educational toy, socks, etc. - who ever suggested PJ's made a good suggestion. I buy soooo any clothes for my kids, but rarely ever buy pajamas. Right now my kids have one pair of pj's each. They grow out of them so fast!
I like to do a themed gift. I will put together a bath basket from registry and non-registry items or a goodnight basket or something like that. I just think it's cute.

A basket with necessities people don't think about. A health kit with nail clippers and the nasal bulb, a thing of saline (to go with the bulb), Tylenol, teething tablets, diaper cream (the Butt Paste is good) and pacifiers (if they registered for them), plus a small thing of diapers. Usually a size 1, NOT newborn.

I buy the baby bathtub they registered for and a bunch of the little stuff - the boring stuff that no else bought (baby wash, tylenol, whatever), and maybe stuff I liked but they didn't register for (nose frida and triple paste).
I also get a blanket or towel, line the bathtub with that and put the little stuff in it and make a gift basket out of it.

I usually buy something off of the registry. If they don't have much left, I go with a bath soap set and some diapers. I usually go with something more practical than fun.
I usually go with the basics that moms use constantly--onesies, baby blankets, washcloths, bibs, etc. in the softest materials I can find that are washable.
In most cases I buy off the registry and sometimes I'll include something else they didn't register for that people rave about (or now that I have DS stuff he loved). Since she is young and registered at walmart, I would probably get her something nicer as well. I always include a gift receipt. And if you get diapers-make sure you give a gift receipt. DS had blowouts with huggies, so we only use pampers. I gave away a few of the huggies we got.
As a side note, some really useful stuff we got was the FP seahorse (belly lights up at night and plays music. DS loved this from 4 months on. Sophie the girafee was a great teether. We used tons of miracle blankets and swaddle me's the first 4 months (not sure if walmart has those). I got a few granimals onesies from walmart and they were crap. They are thin, run very small and shrink. I would get Carters (at the store or online, they always have coupons) if you get some of those.
I'm a lurker popping in with a few ideas. Some of the greatest gifts I got were the most boring things: bably nail clippers, teething gel, teething tablets, infant pain killer, travel toiletries like baby powder and shampoo and diaper rash ointment, travel hand sanitizer, dirty diaper bags, travel baby wipes, etc. Things you don't think of getting but are so glad that someone got them for you.
