I agree, but I also probably wouldn't care if they did. I have girl so not really applicable, but they will do their hair every once and a while, I wouldn't care if they put hairspray in after they did

I think the teething product, corn starch and hair/nail items are not necessary. Would there be a reason for the cosmetics? (A play or dress up or such??)
What about Tylenol/ibruprofen? pepto? Calamine lotion? - as my boy is covered in poison ivy and my one girl has 10000000000 mosquito bites.

I would say no to all of the things the others have indicated they would say no to as well.
Our baby has super sensitive skin, too, so if your daycare provides things like sunscreen and insect repellant, I'd find out what kind they use and make sure you're okay with it. (I'm anal about this from a chemical perspective, as well. We have to provide our own at our center, though, so it's a moot point.)

I would only allow permission to administer sunscreen.
The rest of them at 8 months of age I would give them my WTF face for. Teething tabs are the way to go anyway and you can fill out a form for that when the time comes and they should be the kind you buy and not whatever the daycare has laying around.
I'll have to ask the DCP this, but should I expect to get a new form after the authorization date has passed? What should I put down as the date for the authorization to end since when E is older, he will probably have different needs?
ETA: E will be 8 months old when he starts in the fall.
We fill out new forms each semester. I'd expect to redo them at least once a year. And if needs change before a new form is filled out I imagine you could fill one out at any time

It might be easier for you to get a list from the DCP of what they use (for everything), and then decide if they're okay with you. For example, at my daycare they use the same sunscreen that we do and a bug spray I have no issues with.
That will also give you some assurance if they DO slip up and, for example, use their sunscreen instead of yours once, that it's something you know about. If it's a good center, they're going to try, but they may make a mistake now and again even with the best of intentions. Especially if your preference is a preference and not something allergy based.

You can put your own notes on this form. For example - sunscreen and bug spray - you can check yes and then put a star and a note that says "only what has been provided by a parent may be applied."
I also put "until revoked" for the expiration date.
For things that you may be on the fence with you can check "yes" and then add the caveat "only if permission is obtained by phone prior to application." This way - in general, no they can't put it on your kid. But if there is something going on that merits putting it on your kid, they can call you and you can okay it and they have the written permission for their files and everyone is happy.
You can also just write a letter and kind of make your own form if you want more specific instructions.

I am not sure about what to fill out. My daycare makes me fill out a form for everything they need to apply (diaper cream, aquaphor, etc.) and every medication (albuterol and lotrimin so far) I fill out a form and need a doctors note. They are really strict about it. Personally I want to know what they are putting on my baby just in case there is a reaction. However, he has scratched himself a few times and they wash it with soap and water and put antibiotic cream on it and I am okay with that.

This made me laugh I am just imagining my poor baby surrounded by stoned cats. I will have to look into it though. Just worrying about the sunscreen was enough anxiety before our beach trip I didn't want to fall down the bug spray hole. I am so torn between CHEMICALS and skin cancer, mosquito diseases, etc. there really seems to be no right answer.

I use the honest co bug spray on DD. When she was younger, I sprayed it on her clothes. Now I spray her limbs (mainly her legs) and her clothes. It's a little stinky but does a decent job for just typical outdoor play.
I would refuse the baby powder for sure. You aren't supposed to use it anymore because the particles float in the air too long and are inhaled too easily.
ETA: I would ok diaper cream if you use disposables. DD seems to go from 0-60 with diaper rash.
