We had it catered from a local grocery store--food was great and really cheap, since breakfast costs less than most other meals. We had egg casserole, bacon, pancakes (and toppings!), fruit salad, hash browns, donuts, and cake. I also had a mimosa station set up for adults, and chocolate/regular milk boxes for the kids.
For favors we did Skip Hop water bottles since we he had a few older kids coming, too.

For my girls' family party one year, I did "breakfast for dinner" since that's their favorite meal. Pancakes, waffles, quiches, coffee cake, sausage, bacon, muffins and quick breads, french toast casserole, fruit, milk and various juices to drink. Nothing fancy beyond the menu...I didn't ask the grandparents and such to wear PJs (but I love that idea for kids) and we don't do favors for the family.
If you want to mix things up on the book favors, Boynton's "The Going to Bed Book" was always a huge hit in my house. Or bags with cookies and milk (the shelf stable boxes), like a bedtime snack?

The party I threw wasn't breakfast themed, but I have served brunch at both of M's parties. I made quiches, muffins, and a fruit salad. My friend made a green salad. We had juice for the kids, and mimosas for the adults.
