DH and I are celebrating our 10th anniversary in Oct. We originally planned to go to Ireland but DS started school (we thought we would end up waiting another year but his preschool teacher said he was ready). So now we don't know what to do. My dad would have watched him but he lives in Pittsburgh so we are going to have our sitter watch him for one or two nights. We are in Harrisburg so we can do any of these cities pretty easily. DH was just in NY so I don't know if he will really go for that. He wanted to go to the hershey spa and dinner. Looking for some fun stuff and also something maybe a little romantic. He was just in NYC for work so I think that will be the hardest sell. Also feel free to recommend your great spots for craft beer. DH has just started collecting beers to drink and also age so I'm sure that will be part of the trip.
NYC beer: The Ginger Man, Rattle n Hum, Blind Tiger; Brooklyn: Brooklyn Brewery, Torst, Barcade, Spuyten Duvyl (sp?), Fette Sau (awesome bbq, too). Pick up bottles at the Whole Foods down on Bowery.
Philadelphia beer: Monk's Cafe (awesome Belgian mussels), Eulogy Tavern, Tired Hands Brewery, Barcade, Khyber Pass
Baltimore beer: Ale Mary's (I haven't been to Baltimore in a while). Orioles tix are cheap.
You can do more research on the Rate Beer and Beer Advocate websites for pubs, breweries, and bottle shops.
Eta: oh and there's a good liquor store near Bethlehem called Shangy's.

Baltimore! . Harbor East is beautiful. Lots of romantic restaurants and nice hotels, all very walkable. Wit and Wisdom in The Four Seasons I hear is great, though I haven't been. Pazo is a tapas place right by there. Cinghale is a delicious Italian place, right there. Like PP said, Ale Mary's is right there too.
The Fells Point waterfront is very nice as well, and a much older area of the city.
Woodberry Kitchen is one of my favorite restaurants ever, that's in Hampden. Birrotecca is right near Woodberry Kitchen and makes their own beers. Union Mill Brewery is also right there.
In Baltimore, near Harbor East, is also Heavy Sea's. Good beer and good food.
Of the 3 - NYC is obviously HUGE and for a quick trip could be overwhemling. Almost too much to do. I don't really know Philly. But b'more, as everafter mentioned - there is a lot to do downtown. I think Harbor East/Fells Point are great areas to roam around, try different restaurants, etc. And there are lots of good beer options.
I am biased toward Philly, although we did NYC for our 10th. I have no real experience with Baltimore.
I would maybe check out groupons or living social for philly: Baltimore-- I'm always seeing tours or events that I didn't know existed. Maybe you'll see something that strikes your fancy and you can build a weekend around that?
How about New Hope/ Lambertville? There are some beautiful b&bs, cool shops, great restaurants. Not as much craft beer, but there are wine tours in the area.
For beer in baltimore, I would 100% go to the Brewer's Art. Awesome beer, fabulous food (nice restaurant upstairs, very, um, atmospheric, bar in the cellar downstairs). it's prob my favorite place to get beer in Baltimore. But be careful, because some of those will knock you on your ass before you know it.
Fells Point also does a walking ghost tour in October that would be really fun.
For a weekend, in general, I would do Baltimore or Philly, just because those cities seem more manageable for a weekend and are closer for you than NYC, right?

River Horse is there, but I wouldn't make a special trip for that. Kane and Carton are good Jersey Shore breweries that aren't unreasonably far away.
Warwick NY has a ton of B&Bs and wineries. Captain Lawrence Brewery might be within driving distance of there.
MH said he liked Cape May Brewing. Idk what Cape May is like in October, though, but it's a neat little town. You could drive your car onto the Cape May-Lewes ferry and drive a few more minutes to Rehoboth Beach for the Dogfish Head brewpub.

Riverhorse moved to Ewing last year, which is 20-30 minutes south. Triumph is in New Hope. Village Idiot is in Mt. Holly and Flying Fish is down near Cherry Hill. The Vault is in Yardley and there's Free Will also in Bucks Co. If you were interested in NJ/PA border area.
