Pacifiers, Bottle nipples, onesies, some more swaddlers (mostly because those were ruined by DS#1), and more hats and those little mittens because I lost them.
My kid are 19 months apart, a boy and girl. Poor D wore her brother's hand me downs for years, but I did buy some girly stuff for her to wear, too. They were born in opposite seasons, too, so often the hand-me-down clothes that fit her were not suitable to the season.
We purchased a double stroller, we needed a new supply of teething toys when the time came (S was a big chewer, and pretty much destroyed all the teething stuff), we purchased a bed for S when D outgrew her co-sleeper and moved into the crib, we purchased a little support ring-type thing that helps them sit upright in the bath so that they could take baths together after D turned 6 months or so.

Another crib and mattress. Double stroller. Lots of clothes. DD2 was born in Jan and DD1 in Aug. So a lot of the clothes weren't useable due to season.
Clothes and crib sheets (bc my 2 kids are boy/girl)
I can't really think of anything else. I made sure most of the staples were gender-neutral the first time, and my oldest hated all play mats/ exersaucers/ bouncy chair/ pack and play, so all that stuff was only very very lightly used the first time around.
Car seat. Because the first one was only sixteen months older and still needed hers. Anything that went into the mouth (teethers, nipples) primarily because of use. Stuffed animals and special blankets (so they'd have their own fuzzies and lovies.)
We bought a new bouncer seat thing because we borrowed one with #1.
Thinking about getting something to serve as a bassinet since we don't have a ton of room next to our bed for the pack n play we used last time. But I'll probably cheap out and squeeze it in there anyway.
Newborn insert for the Ergo carrier since I never got the hang of the Moby wrap and preferred the Ergo anyway.
Bottle parts
A new, warmer coming home outfit.
An extra Boppy cover.
Miracle blankets if I can't find the ones from last time. They have to be packed away. Have to!
Depending on the sex of the baby, we might need new clothes. We might need new clothes if it's another boy anyway since they'll be born in different seasons. I'll play that by ear though.
Oh, and after much internal debate, I passed on the RNP. A friend let me borrow an additional PNP, which we set up in our room and put him in at night for the first 2-ish weeks. After that, he's been in his crib in his room. I'm kinda glad I skipped it because we just wouldn't have gotten the use out of it.
New carseat (DD's was too old), crib/bedding, clothes (boy the second time around), and I got an Ergo and a better sling this time around. Otherwise, I think that was about it for him.
Quite a lot actually because I didn't have a lot of stuff with my first. She was an easy baby and DS was not. So I kept buying things hoping something would make him stop screaming lol. Also I had such a gap between kids stuff was better.

RNP - because we didn't have one with DS1 or DS2. Bottle nipples for DS2, didn't have to replace for DS3 because he's never had a bottle. A bigger/better/more functional diaper bag. After 2 kids I knew what I wanted and needed much more than when I just chose one while pregnant with DS1. Socks. I still used some old ones (what I could find) but I've had to supplement socks for DS2 and DS3. I bought A&A swaddle blankets for DS2 (and used them with DS3, too) because we didn't have any with DS1.

Baby book, obviously Some friends got me the rock 'n play sleeper and man, I wish they'd had those when DS1 was born.

We will have to get a new crib/mattress, clothes (maybe, they'll be different seasons), rock n play, stuff for baby wearing (I lent my moby wrap out and I don't think I'm getting it back), bottles, pacifiers, etc.
My parents are insisting on getting a new infant seat and since they're buying new a nice one in not gonna argue with them lol.

We replaced the crib mattress and sheets, bottle nipples, and my mom got me a Baby Bjorn b/c I wanted a carrier (we didn't have one from #1 b/c I tried it with a borrowed one and she HATED being worn...but with two kids I needed my hands so #2 was going in there like it or not and fortunately she loved being in there). And once she graduated from the infant carrier at 6 months old we got another car seat since #1 was still using hers.

DD and DS are 5 years apart so I had to buy everything new. I got rid of all her baby stuff years ago because at the time we were one and done. The only thing I had saved was a baby food steamer/blender and her baby clothes because a friend wanted to use them as hand me downs. It was nice being able to buy all new stuff because a lot of cool baby gear has come out since I had my first.

Crib, crib mattress, changing pad, and some more crib sheets. Not much else. He'll get a new convertible car seat soon, but for now he's still in the infant carrier.
