Physical Therapist: I would probably do homemade cookies or candy and a nice card
Cleaning service: I think "doubling" is a bit much for cleaning service, so I usually do a gift worth about $50 (we usually do a gift card to someplace like Target).
I do a larger tip than normal for my hairdresser whenever I get my holiday cut/color....like I double my usual tip.

I'd give Target gift cards to the temp nanny and PT at whatever amount fits your budget. That's very generous for the cleaners, but if they're good, it's probably worth it. I get my hair cut 4x/yr. I give a normal decent tip every time. I don't tip more at Christmas and I don't know anyone that does (I don't live near a big city and would consider us MCOL)
ETA: I agree with pp about the cleaners. I don't know how much you pay, but double is a lot.
For a temp nanny I might do 1/2 the week's salary thing.
Physical therapists are a different matter. They're are highly trained and educated professionals not service industry people. A few might find this insulting. If this is an EI agency PT, the agency may have guidelines. Naked cash wouldn't be appropriate, but a gift card might be OK.
I wouldn't do a holiday tip for a first appointment, but I like to avoid awkward, so I'd make the appointment sooner rather than the week before Christmas.
Oh, yes, I know! I just ran out of characters in the title of the post. She's great with DS so I want to recognize her and say thanks without being Scroogey or inappropriate.

Nanny: cash, no matter if it's $20 or much more. PT: a small gift card to Starbucks ($5-$10) with a card from your kid No idea on cleaners. I wish I had one to figure this out. New hair person, I'd just tip regular.
I'm trying to figure out cleaners. I don't know how many of them come. I normall pay $210 and I was thinking of leaving $100. But if there are 3 of them maybe I should leave $150? I guess I should email to find out. I never see them and have only met the owner.
