I have a friend whose spouse I don't particularly care for and I want to send a personalized Christmas card. Do I have to write it to both my friend and the spouse and write something more broadly generic or can I address just my friend?

I would address it to the family on the envelope, and write the personal note to your friend on the card. As long as you aren't planning on writing about how you don't like her H, he won't care about the personal note.
Well, as many people don't send Christmas cards at all - I don't know that I really subscribe to any hard and fast etiquette rules if I do decide to send cards.
Meaning - if you want to send a card to just your friend, then do it.
