If after 2 years your manager didn't know how to spell your name.
Imagine your name was typically spelled with a G but you spell it with a J, which makes sense given your full name. And your boss always uses the wrong one.
I'd be annoyed if it was direct supervisor. My dad spells my daughters name wrong every time and she is almost 7. It is Peyton and he spells it Payton. Drives me crazy!
I'd have hit rage by about month 7. By 2 years I'd have circled all the way back around to finding it absolutely hilarious and would entertain myself as frequently as possible with it. Like, every time I wrote a name tag I'd put the letter s/he gets wrong in a different color. Or I'd put that letter in a larger font in my email signature. Or I'd start spelling his/her name slightly wrong in emails every so often.
My old boss's boss called me another name entirely for 6 months.
This is me. For 8 years he kept confusing me with another coworker whose name also started with C. We looked nothing alike, we acted nothing alike, we HATED each other, and we wore ID tags all the time with our full first names printed in very large letters. Still, he was forever confusing us. It only stopped this year because she transferred to a different campus.

My last name gets spelled by the staff at my school incorrectly all the time. There's an E in our last name that you don't hear so people leave it out and then since is a 'son' last name that spell it son but ours isn't spelled that way. Drives me crazy. I always just politely point it out, because 90% of the time if I don't I end up having to spell my own name wrong for the remainder of the school year to be able to access things. I'm say something like this to your boss " My name is g however it is spelled jxxxxx not gxxxx. Just thought you should know so that paper work can be done correctly."

My entire name is in my email address. In order to send me an email, you have to spell it correctly. 85% of the people at work still start the message with my name spelled incorrectly. I stopped being bothered by it, but only because I would always be ticked off if I let it bother me.

I'm in the same place - my direct supervisor puts a space in my name, even though my email signature clearly has NO space (think JoAnn and Jo Ann). Even worse, my mom made my name up completely, so it's not like anyone has seen it spelled another/more common way.
ETA: after 8 years with my H, FIL still spells my name wrong. For Christmas, he gave us both t-shirts signed by Garth Brooks...with my name spelled wrong

I did this with a holiday card this year. I didn't change my name. My mom's friend has trouble accepting this. She will address me as "Cville Cvillehusbandlastname" no matter how many times I correct her. She also doesn't like her own grandchild's first name so calls the kid by the middle name. This is an ISSUE with her in many arenas.
Anyway, we have a return address stamp that says "CvilleLN & CvilleHLN" that we usually use. For the card going to her I wrote it by hand. Like so:

Totally different but I just remembered- at my old company, everyone referred to me as Jay-Z. There was one man I didn't work closely with but we were friendly. I didn't realize until he left me a note about something one time that he thought my name was Daisy the whole time. Lol! He'd been saying "Hi Daisy!" For so long and I didn't even notice. It was so awkward once we figured it out.
Oh I just remembered another one! Back when we were dating, I got a copy of H's graduation announcement in the mail. I knew his middle name was Manuel, so I was sort of surprised to see it spelled "Manual" on the announcement. When I asked him about it he just rolled his eyes and said that his mom had been making that same spelling mistake for years.

I'd be annoyed. My name is spelled a little different and I always get work emails with my name spelled wrong. If it's from a parent (I'm a teacher) I just sign it with my name spelled right and don't get too offended if they reply with it spelled wrong again. If the email is from a work mate, I sign my name with the different letter capitalized. If it happens again, I usually spell their name wrong and then they usually get it. :-)
I'd be annoyed but I am really good with names so when someone doesn't bother to be good with my name, it seems like slap in the face to me and makes me feel I don't matter. Like, remembering names is SO easy for me it is hard for me to remember others aren't like that.
I was a nanny for a family once for a couple of years and after I'd been there almost a year the dad had to write me a check. He started to write it and was all "Uh...how do you spell your first name?" Ok, my first name is not common but still. Then "What's your last name?" DUDE. Maybe know the name of the person that has access to your house and kids all day. I could steal your kids. Good luck with that call to the cops. My nanny has my kids! Ok, sir, what's her name? Uhhhh...I don't know.

I gave up at my job. For whatever reason randomly my name would be spelled with a k instead of a c, and my last name would be spelled phonetically rather that it's proper spelling. I've worked here almost 4 years! In all fairness they seem to get even their own names wrong sometimes.
I would be annoyed if I was the only one and it happened after being corrected. I will admit I sometimes misspell the names of the employees I manange. Like a double letter that is common but that person doesn't use.
