Outside of the content, this is a terrible infographic.
Right? The whole point of an infographic is to be scannable, easily digested bits of information in a visually pleasing way. This is, well, none of that.
A friend of mine was bitching on FB because SHE HAS THE MUMPS. She laments the fact that for some unknown reason, her mother never fully vaccinated her. Her grandfather was a physician and falsified her vaccination record so she could attend school. True story. Mumps apparently suck.

What? Like they just forgot and then she was headed to school that day and so her mom asked her own dad to just sign the papers instead of giving her the mumps vaccination right then? So wacky and I hope your friend recovers quickly!
I did a quick Google search of her name just to see if there was any more idiocy around and stumbled across this blog post: scienceblogs.com/insolence/2014/04/23/criticism-is-not-hate-speech/ Apparently, we're far from the only ones who have her number. This person not so succintly explains in detail why she's an idiot.
The best part is at the bottom (and it's a loooong post, but scroll down; it's worth it), when he gives a brief description of how her blog was taken down because of that ultra conspiracy group. It's great just reading it and remembering how hilariously crazy they were.

No, like her mom decided against completing her vaccinations but NYS is really strict about opting out, so she got her dad to sign off on it as though it had been completed so she could attend school. Shady, shady.
