After seeing the post about NQB's well visit, I was somewhat surprised by ours today. We didn't do an autism screening or fill out any other questionnaires that I have seen others talk about. I'm not concerned about DS' development, but I just thought it was unusual. Our pedi just asks basic questions about his speech and fine/gross motor, but nothing other than that. Is this normal? Would it concern you at all? FWIW, I love the practice and have had no concerns or issues.
I have never filled out a comprehensive questionaire like nqb did. I have done a general autism screening one a few times now for J. It's just a short circle yes or no type thing to about 20 questions. I don't think it would concern me if this werent done. I think the questions on there are fairly obvious and as a parent I would already be aware of the behavior and bring it up wth my pedi if I noticed it. That's not to say everyone would know better so I do think they can serve a purpose, but as for me personally, it wouldn't bother me not to do it because I am aware if that makes sense.
The only questionnaires we have are the laminated ones in the office asking us if we can read the survey and questions related to vaccinations. She just observes and asks a few questions during the appointment without any formal questionnaire. We have parents as teachers and had early intervention services and I do/did sometimes like the questionnaires because it told me how on track he is with development.
I have a questionnaire at every appointment FWIW. I think it's probably because it's a larger practice and I won't always see the same doctor so maybe it's more of a CYA.
Both clinics I've been to with DD (we moved and switched clinics) has an early autism screening questionnaire at her 12 month, 18 month and 2 year. Then when the doctor comes in s/he asks a lot of the same questions to check DD's development. She was done getting all her shots at her 18 month appt. (besides yearly flu shots) and won't need anymore until she's 5. Every place and clinic is different though and if I liked the clinic and doctor otherwise I wouldn't worry.
We do our vaccines with the Public Health nurse and at every appointment we have had to fill out a questionnaire about development. At their well-check visits with the doctor we have never filled out any kind of survey, the Dr just asks general questions about development during the check up.

We get an ages and stages form to fill out. It's kind of nice because we get it for the next age, so we have an idea if what kind of skills we should be expecting our kid to do.
I thought there were 18mo vaccines? At least the nurse yesterday said there would be? (somehow C is missing a hep one and said she'd get it with the others at 18mo).
DS got his hepatitis A shot and they also pricked his toe to check his iron.
I'm sure most doctors can catch any red flags with just a couple observations and casual questions (most parents probably would too). My sister is a special education teacher and says there are very easy cues she sees in DS and our niece that make it obvious they are not autistic. I know it's a full-spectrum disorder, but I'm just saying they probably "test" in roundabout ways.
