I don't remember the safety recommendations and I'm wondering what a normal age to add pillow and/or blanket is? Did the addition of pillows/blankets coincide with switching to a bigger bed, or did it come earlier? Just curious!

I don't know what's normal, but we bought DD a pillow (a small one from IKEA) at around 13 months. We still dress her in a sleep sack though because it's winter and I'd assume she'd just kick that shit off right away since she does move around a bit while she sleeps.
DD at 3.5 still kind of hates pillows. She didn't get one until she was in a bed at 2. She got a blanket right about a year in the crib when she started dragging it with her.
DS at 11 months doesn't have a pillow but he's had a blanket since early early (Like 4 months?) Kid is a terrible sleeper and the only thing that settled him down for a phase was holding a blanket in his fingers and snuggling it up by his cheek.
DD has had a blanket for a while, probably since before a year. We usually go in and cover her before we go to bed. We don't have a pillow yet, but have wondered about getting her one. But she generally sleeps on her stomach with her legs and hands curled under her and her butt in the air. Not sure how comfortable a pillow would be with that position.
I recently added a pretty flat pillow & knit blanket to DD4s bed (at 20-21mo old). I think she sleeps much better now vs no pillow & a sleep sack. She stays put on the pillow & under the blanket which honestly really surprised me.
We gave DD a pillow in her crib shortly after her first birthday and gave her a blanket once she outgrew the sleep sacks she was in, so before 18 months. She was napping at my in-laws with a pillow and blanket before that and seemed to prefer sleeping that way. We moved her to a regular bed the week after her 2nd birthday. She still doesn't stay on her pillow or under her blankets.

I added a pillow for DD when we moved to her twin bed. I'll do the same for DS. We gave blankets shortly after a year. DD didn't really sleep under (and stay under) a blanket until she was around 4. She's all over the bed when she sleeps. DS on the other hand loves blankets.
